FHIR scheduling APIs across EHRsCustom medical practice management software
We build practice management software for specialty groups, MSOs and health tech products: scheduling across providers, rooms and locations, real-time 270/271 eligibility before the visit, charge capture from the encounter, 837P and 837I claims through your clearinghouse, 835 ERA auto-posting with denial queues, patient statements and payments, and two-way integration with your EHR. The same system gives owners and billers the reports they run the practice on.

The short answer
Practice management (PM) software runs the business side of a medical practice. It books appointments across providers, rooms and locations, registers patients and their insurance, checks eligibility with an X12 270/271 before the visit, captures charges from the encounter, sends claims as X12 837P or 837I through a clearinghouse, posts payments from 835 electronic remittance advice, works denials, bills patients for their balance and reports on the revenue cycle. It sits beside the EHR, which holds the clinical record, and the two exchange patients, appointments, encounters and charges. We build custom PM software when an off-the-shelf system does not fit the specialty, the number of entities or the product being sold.
Read our X12 developer guide: 835, 837, 270 and 271Start with the real problem
A visit is booked for the wrong plan, eligibility is checked after the patient leaves, a procedure is charted but never charged, a claim is rejected at the clearinghouse and sits unseen, and an ERA posts to the wrong line because two visits share a date. Front desk, clinicians and billers each work in a different screen and re-key the same visit. Good PM software treats the visit as one record that moves from appointment to encounter to claim to payment, with a queue for every exception and a person who owns it.
What we build
Most engagements build three or four of these modules and integrate the rest with systems you already use.
Appointment types, provider templates, rooms, equipment and locations, with online booking, waitlists, reminders and referral prerequisites checked at booking.
FHIR scheduling APIs across EHRsDemographics, guarantors, primary and secondary coverage and card capture, with real-time eligibility and benefits before every visit and prior authorization flags.
Automating 270/271 eligibility verificationCharges created from the signed encounter with CPT, HCPCS, ICD-10-CM and modifiers, fee schedules per payer, and edits that catch missing or mismatched codes before billing.
Claim scrubber software buildProfessional and institutional claims built, scrubbed and sent through your clearinghouse, with acknowledgments and claim status matched back to each claim.
Clearinghouse integration production guide835 remittances auto-posted to the right claim lines, adjustments classified by CARC, RARC and group code, and denials routed to a queue with the payer's reason attached.
Payment posting automation with ERAPatient balances, statements by mail, email and text, card on file and payment plans, Good Faith Estimates for self-pay patients, and reports on A/R, collections and denials.
RCM software developmentYour starting point
Your specialty's scheduling, coding or payer rules do not fit a generic PM system, and staff work around it in spreadsheets. We build the modules that do not fit and integrate the rest, or replace the system in phases.
You manage scheduling and billing for many practices on different EHRs. We build a multi-tenant PM platform with per-practice fee schedules, payer enrollments and reporting, and one clearinghouse connection.
You sell software to practices and need scheduling, eligibility, claims and ERA inside your product. We build the PM layer and the payer connections as custom modules you own once paid, and ship.
The Engineering Engagement
Workstreams are selected around your priorities. Each comes with an output your team can inspect, test and own.
Appointment types, durations, provider and room templates, overbooking rules and cross-location availability, with concurrency protection so two users cannot book the same slot.
Demographics, guarantors, authorized representatives and insurance, with duplicate detection that routes uncertain matches to review.
Real-time 270/271 at booking and again before the visit, with copay, deductible and coinsurance shown to the front desk in plain terms.
Charges from the signed encounter, payer fee schedules, scrubbing edits, and 837P or 837I claims with 999, 277CA and 276/277 handling.
835 auto-posting, denial queues by CARC and RARC, secondary claims, patient statements, payment plans and online payments.
Two-way exchange of patients, appointments, encounters and charges with Epic, Oracle Health, athenahealth, eClinicalWorks, NextGen or your EHR over FHIR, HL7 v2 or vendor APIs.
Days in A/R, clean claim rate, denial rate by payer and reason, collections, provider productivity and no-show rates, from one reporting store.
Compare your options
Each option runs a practice. The right one depends on your specialty, how many entities you run and whether PM is part of your product.
| Custom practice management software | Off-the-shelf PM system | PM bundled with your EHR | |
|---|---|---|---|
| Best for | Specialty groups, MSOs, billing companies and PM products | Small practices with standard workflows | Practices standardized on one EHR |
| Specialty workflows | Built around your scheduling, coding and payer rules | Configured within the vendor's options | Configured within the EHR's options |
| Multi-entity | Tenants, fee schedules and payer enrollments per practice | Varies by vendor and license tier | Usually one organization |
| Integrations | Your EHRs, clearinghouse and payment processor | Vendor's integration list | Native to the EHR |
| Data ownership | Your database and schema | Export terms in the contract | Inside the EHR |
| Cost shape | Build cost, then hosting and support | Per-provider subscription | Part of the EHR license |
Many groups keep the EHR's PM for standard work and build custom modules for the specialty or multi-entity parts it cannot handle.
Expertise is in the decisions
Keep the incumbent PM where it works and build the modules it cannot handle, or replace it in phases by location. We compare both, including migration of open A/R, balances and appointment history.
Decide whether the PM or the EHR confirms, cancels and reschedules visits, and where the charge is created. Each record has one owner and the other system receives updates.
The clearinghouse contract, EDI enrollment and ERA and EFT enrollment with each payer take weeks. We start them at kickoff so claims and remittances are ready when the software is.
Fee schedules, scrubbing edits, payer rules and statement cycles change often. Store them as versioned data your billing team can update, with tests that run before each change goes live.
Code selection, modifiers and payer-specific interpretations are set by your certified coders and billing leads. The software applies and versions the rules they approve.
Card numbers are tokenized by your payment processor and never stored in the PM database, which keeps PCI scope small.
Timeline
The number of modules, EHRs and locations, and payer enrollment lead times, set the schedule.
Observe booking, check-in, coding, claims and posting with each role, and list the systems in play.
Scheduling, registration and eligibility, or the billing modules, depending on where the practice loses most time.
Charge capture, 837 claims, acknowledgments and 835 posting through your clearinghouse.
More locations, patient billing and reporting, with migration of open balances.
Delivered work
Client names are withheld; each is a real engagement. Our open-source X12 work is public on GitHub.
Scheduling, charting, treatment plans, insurance and patient splits, invoicing and double-entry practice accounting in one system, with each clinic's data isolated.
Eligibility before every session, claims checked against payer rules and sent through the clearinghouse, and denials returned to a queue with the payer's reason attached.
Coverage checked through the clearinghouse before each visit, the benefit response read into plain terms for staff, and every claim run through CPT, modifier and NCCI checks.
Scheduling, charts, care plans and billing where every record, including appointments, slots and invoices, is a standard FHIR resource.
A test clearinghouse for 270/271, 278, 837, 999 and 277CA, 835 and 276/277, used to test claims and remittance flows before a live connection.
From discussion to delivery
Booking, check-in, charge entry, claim submission and posting mapped with each role.
Queues for eligibility failures, missing charges, rejections and denials designed with the people who work them.
Working software every two weeks, tested against real EHR payloads and our clearinghouse simulator.
Controlled rollout with training, reconciliation reports and a support path.
We agree the scope, dependencies, acceptance criteria and commercial model before implementation. Your existing team can stay involved throughout.
Find the right starting point ↓It is the software that runs the business side of a medical practice: scheduling, patient registration, insurance and eligibility, charge capture, claims, payment posting, patient billing and reporting. It works beside the EHR, which holds the clinical record.
The EHR holds clinical information: notes, orders, results, medications and problems. PM software handles appointments, insurance, charges, claims and payments. They exchange patients, appointments, encounters and charges, and some vendors sell both in one product.
A scheduling engine, registration and insurance, real-time eligibility (X12 270/271), charge capture, a claims engine with scrubbing (X12 837P and 837I), a clearinghouse connection, ERA posting (X12 835), denial management, patient billing with a payment processor, EHR integration over FHIR or HL7 v2, and a reporting store.
Two-way exchange of patients, appointments, encounters and charges; support for FHIR (Patient, Appointment, Slot, Coverage, ChargeItem) and HL7 v2 (ADT, SIU, DFT); vendor APIs for Epic, Oracle Health, athenahealth, eClinicalWorks and NextGen; and reconciliation reports for messages that failed.
It sends an X12 270 eligibility request through the clearinghouse and reads the 271 response for active coverage, copay, deductible, coinsurance and prior authorization requirements. We run it at booking and again before the visit, and show the result to the front desk in plain terms.
The 835 remittance is parsed, matched to the claim and service lines it pays, and posted with its adjustments classified by CARC, RARC and group code. Payments are reassociated with the EFT deposit, denials go to a queue, and secondary claims are created where a balance remains.
Yes. We build claims engines that create 837P and 837I claims from charges, scrub them against payer and NCCI edits, send them through your clearinghouse, track 999, 277CA and 276/277 responses, and match 835 payments back to each line.
Buy when your workflows are standard and one vendor covers your specialty and EHR. Build when your specialty, number of entities or product needs do not fit, when you run many practices on different EHRs, or when PM is part of what you sell. Many groups build the modules that do not fit and integrate the rest.
The first modules at one location are planned in months, with claims, remittance, patient billing and more locations following in phases. Clearinghouse and payer ERA and EFT enrollment take weeks, so we start them at kickoff.
The number of modules, EHRs and clearinghouses to integrate, the number of practices and locations, specialty rules, data migration of open A/R and balances, and reporting needs. We give a fixed-scope estimate after discovery, broken down by module and integration.
Yes. We build Good Faith Estimates for uninsured and self-pay patients under the No Surprises Act, and patient statements by mail, email and text with online payment, card on file and payment plans through your payment processor.
Yes. We sign BAAs and we are ISO 27001:2022 certified. PM systems are built with role-based access, audit logs, encryption in transit and at rest, and card data tokenized by the payment processor.
